The Gift of Care Program

One Individual per 30 days, who meets the following criteria, may be eligible to receive up to 50 hours of non-billed, non-medical home care services through Abacus Home Care under the direction of the Pennsylvania Foundation for Home Care and Hospice (the “Program”):

• Individuals who temporarily or permanently reside in the Commonwealth of Pennsylvania;
• Individuals who demonstrate a need for care in their home, which can include assistance with ADLs, companionship, and/or respite type non-medical home care services;
• Individuals with monthly income less than $5,000 (single) or $10,000 (dual income, including spouse/partner, excluding any child income);
• Individuals who are not currently receiving, are waiting to be approved/renewed, or are not eligible for comparable in-home services through the following programs:
  • Any Pennsylvania Medicaid Waiver Program (including managed care programs);
  • Veterans who are receiving home health care benefits through the “Aide and Attendance Program”, or any other program as deemed similar by Abacus Home Care;
• All referred individuals are approved at the sole discretion of Abacus Home Care;
• Nothing in the program creates any guarantee of obligation on the part of the Abacus Home Care to provide home care services to any individual.

Abacus Home Care will accept one referral per 30 days for individuals with the highest need level (the “Recipient”).

• Send all qualified referrals to GiftOfCare@AbacusHomeCare.com for consideration. 
• Once approved, the recipient will be granted access up to 50 hours of non-billed, non-medical home care services (the “Services”). Services shall be delivered between the hours of 8:00 am and 12:00 pm, Monday through Friday, for a period of two consecutive weeks (the “Term”). Any hours that are not used during the term will be automatically forfeited by the recipient.
• Abacus Home Care must receive referral applications no later than the 25th of each month for consideration into the program for the following month. A new recipient will typically commence the services each month on the 1st Monday of the following month.
• In an effort to maximize the number of individuals enrolled into the program each year, recipients are expected to commence services within 10 days of signing the Service Agreement.
• Abacus Home Care will remove selected recipients from the program, if the recipient:
  • is hospitalized, transferred to a skilled nursing or personal care facility, independent living community, or other similar scenario;
  • passes prior to the completion of the term; or,
  • becomes eligible for home health care services through a PA Medicaid Waiver Program, Veterans Affairs’ “Aide and Attendance Program”, or through another similar program, services under the program will cease;
  • no longer meets the eligibility criteria for non-billed, non-medical home care services; or
  • chooses to withdraw from the program.
